29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) At the jobsite, Clean and Screen Area - The path to ground from an industrial fan was not continuous and effective due to a missing ground prong. The employees in the clean and screen area used this equipment and manually plugged the cable into an outlet on the wall during the summer months. Observed on or about 06/13/14.
