29 CFR 1926.651(l): Walkways with guardrails complying with 1926.502(b) were not provided where employees or equipment were required or permitted to cross over excavations:  On or about March 20, 2014, at 7640 Harbison Hollow, Lot 91, Suwanee, Georgia:  An employee performing foundation work was exposed to a fall hazard of approximately 7.4-feet while using a metal foundation form that was approximately 8-feet long and 1-foot wide to cross an excavation that did not have guard rails installed.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was not properly sloped, shored or provided with a trench shield.  On or about March 20, 2014, at 7640 Harbison Hollow, Lot 91, Suwanee, Georgia:  An employee performing foundation work was exposed to cave-in hazards while working in a foundation excavation that was approximately 9-feet wide, 7.4-feet deep, and 59-feet long that did not have a protective system installed.
