29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a) Employees engaged in debris and roofing material removal from the approximately 35 foot high roof were throwing the debris and material to the ground exposing the foreman to struck-by hazards, on or about 03/20/14.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Employees engaged in roofing activities on a flat roof approximately 35 feet in height were not using any form of fall protection exposing them to fall hazards, on or about 03/20/14.   b) Employees engaged in roofing activities were working around an unguarded sun roof exposed to a fall hazard, on or about 03/20/14.   c) Employees engaged in roofing activities were working around an unguarded roof hatch exposing them to a fall hazard, on or about 03/20/14.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:  a) Employees entered the building using a set of stairs which were 4 risers in height with no hand rail creating a fall hazard, on or about 03/20/14.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
