On or about March 18, 2014 - at the above addressed jobsite, the employer did not initiate and maintain an accident and prevention program to comply with the regulations of this part, which included but were not limited to, employee protection when working at heights.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a. On or about March 18, 2014 - at the above addressed jobsite, employees conducting roofing operations from a steep roof that measured approximately twenty five (25) feet above the ground, were not trained in the recognition of hazards in their workplace in regards to falls, thereby exposing the employees to fall hazards.

On or about March 18, 2014, at the above addressed jobsite, employees conducting roofing operations from a steep roof that measured approximately twenty five (25) feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about March 18, 2014 - at the above addressed jobsite, employees performing roofing work were exposed to falls when the ladder to the roof did not extend three (3) feet above the landing surface.
