29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. Boiler room - On or about March 19, 2014, employee(s) were required to walk on a concrete floor to access mechanical equipment such as a de-aerator and other equipment. The concrete floor at the de-aerator was not maintained in a dry condition in that the floor was covered with standing water. Employee(s) were thereby exposed to the hazards associated with slips and falls.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. Boiler room - On or about March 19, 2014, employee(s) were required to walk on the concrete floor on a regular basis in order to perform their assigned duties. The concrete floor has a 13 inch deep trench/drain with a metal cover used to blow down the boilers. The metal cover was in disrepair and created a floor hole approximately 6 inches in length and 6.5 inches in width. Employee(s) were thereby exposed to a potential trip hazard.
