29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) Riverfront Park Boathouse Job Site - On or about 3-20-14 - An employee was exposed to struck by hazards from soil falling or rolling from the excavation face while working in the eight foot deep excavation. The shield system was three and a half feet below the top of the excavation.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) Riverfront Park & Boathouse Job Site - On or about 3-20-14 - Employee was exposed to a fall hazard while accessing the excavation from the face which was over three feet in height above the edge of the trench box. The Milton C. Beebe & Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Stairway(s) or Ladder(s) 29 CFR 1926.1051(a), which was contained in OSHA inspection number 314405143, citation number 2, item number 3 and was affirmed as a final order on 8-3-2011, with respect to a workplace located at Bridge Number 40701, Danielson, CT.
