29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   A) Pallet on lift - An employee was exposed to fall hazards of approximately 16 feet while working on a wooden pallet that was supported by a JLG lift. The employee was not provided with a personal fall arrest system or any other means of fall protection on or about March 19th, 2014.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:   A) Back right side of structure: An employee was exposed to fall hazards of approximately 14 feet while using an extension ladder to access the fabricated framed scaffold. The extension ladder that was used did not extend at least 3 feet above the landing, on or about March 19th, 2014.
