29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage:     On or about March 19, 2014; at the center of the jobsite, the employer allowed employees to utilize a flexible extension cord that was not protected from damage.  Employees used the extension cord to power a hand-held band saw.  Employees used the band saw to cut 3 pipe.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On or about March 19, 2014; at the center of the jobsite, the employer allowed employees to utilize extension cords in a manner that was not in accordance with instructions included in the labeling.  Employees connected electrical extension cords together to create a chain of two (2) extension cords in order to power a hand-held band saw.  Employees used the band saw to cut 3 pipe.
