On or about May 20, 2014, employees were exposeed to a fall hazard as they installed thermoplastic polyolefin (TPO) material on the roof of Building #1.  The approximate fall distance to the ground below was 27 feet.

On or about March 20, 2013, employees who used a job-made ladder to gain access to the roof of Building #1 were exposed to a fall hazard.  The ladder extended about 1 foot (0.3 m) above the upper landing at the roof surface.
