29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   A)    FWT, LLC, located at 761 West High Street, Hicksville, OH 43526: On or about 02/25/2014, the employer failed to ensure that the point of operation of the hydraulic Pole Squaring Press # 3 (built by the employer) was adequately guarded. Employees were able to reach into the press point of operation while lowering the ram down and were exposed to pinch point hazards while operating the press.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     A)    FWT, LLC, located at 761 West High Street, Hicksville, OH 43526: On or about 02/25/2014, the employer did not ensure that employees were protected from moving objects such as, but not limited to, large telecommunication towers while employees set the towers in the hydraulic Pole Squaring Press # 3 (built by the employer) to perform the work. The towers were not secured inside the press, exposing employees to struck-by and pinch point hazards.       FWT, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1)), which was contained in OSHA Inspection # 891342, Citation number 1, Item number 1, and was affirmed as a final order on 05/29/2013, with respect to a workplace located at 761 West High Street, Hicksville, OH 43526.
