29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) The employer did not ensure that all employees were trained in a manner in which they were able to understand. For the Deme Construction employees who were working inside a trench on or about 03/20/2014 to expose a 16-inch water pipe to repair a leak. The employees had only been provided brief "tool box" topics which did not address the basics of trenching and excavation hazards. Additionally, the employer's safety manual was only provided in English; not all employees were capable of reading or speaking English.
