29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 03/21/14, at the Parish Commons Medical Building located at 656 Elmwood Ave., Buffalo, NY: A ladder jack scaffold was erected outside on the front side of the building. One employee was on the "pik" of the scaffold and was putting adhesive tape on the sliding glass door on the second level of the building. The employee was exposed to a fall of 12 ft. 6 in. to the ground. No personal fall arrest system or other fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED
