Quality Xpress, Inc. was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.501 (b)(13)), which was contained in OSHA inspection number 786523, citation number 1, item number 2 and was affirmed as a final order on 03/20/2013, with respect to a workplace located at 5 Barbados Lane, Panama City Beach, FL; and OSHA inspection number 671904, citation number 1, item number 2a and was affirmed as a final order on 03/18/2013, with respect to a workplace located at 50 Founders Lane (Lot 58), Santa Rosa Beach, FL.  29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about March 19, 2014, on the top plate, an employee performing framing operations was not protected from an 22 feet fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternate fall protection.   b.  On or about March 19, 2014, on the south side of the house, employees framing the upstairs walls were not protected at the window and door opening from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

Quality Xpress, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number OSHA inspection number 671904, citation number 1, item number 2a and was affirmed as a final order on 03/18/2013 with respect to a workplace located at 50 Founders Lane (Lot 58), Santa Rosa Beach, FL.  29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On or about March 19, 2014, employees performing framing operations were not trained to recognize, control or minimize fall hazards at a jobsite.
