Waste Treatment Corporation - On or about March 20, 2014, and at times prior thereto:  employer did not provide medical evaluations for employees required to utilize 3M brand 8210 particulate respirators during lime pouring operations.

Waste Treatment Corporation - On or about March 20, 2014, and at times prior thereto:  employer did not provide annual respiratory protection training for employees required to utilize 3M brand 7502 tight-fitting, air purifying respirators during tank cleaning operations. Employees were last provided training on, or about, August 3, 2012.    b.) Waste Treatment Corporation - On or about March 20, 2014, and at times prior thereto:  employer did not provide annual respiratory protection training for employees required to utilize 3M brand 8210 particulate respirators during lime pouring operations.  Employees were last provided training on, or about, August 3, 2012.

The Company had not provided employees with effective information and training about the physical and health hazards of the hazardous substances to which they may potentially be exposed.
