29 CFR 1926.404(f)(6): Grounding path. Employer did not ensure the path to ground from circuits, equipment, and enclosures were permanent and continuous. Location: 222 East 104th Street, South Building, 2nd floor on East side. On or about March 24, 2014; a). Employees were not protected from electric shock and electrical burns by a continuous ground path. Employees were utilizing two (2) damaged Flexible cords to energize two (2) portable Flagro electric Floor Heaters. The flexible cords did not have a continuous path to ground because the grounding conductor prong had been removed or broken away from the flexible cords. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
