29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Glove Box Assembly: On or about March 18, 2014 and at times prior thereto, the employer did not ensure the Sonic Welder had a ground wire which exposed an employee to electrical hazards such as but not limited to electrical shock. Kwangsung America Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.304(g)(5), which was contained in OSHA inspection number 314601477, citation number 1, item number 7 and was affirmed as a final order on December 13, 2010, with respect to a workplace located at 217 Thweatt Industrial Blvd.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Paint Room: On or about March 18, 2014 and at times prior thereto, the employer did not establish and implement a written respiratory program to include worksite procedures, procedures for selecting respirators, medical evaluation of employees, fit testing procedures, training of employees in the hazards, proper use, maintenance, and any limitations of respirators which exposed employees to respiratory hazards such as but not limited to obstructed breathing. The employer provided and required use of 3M 8512 N95 Particulate Respirators. b) Grinding Room: On or about March 18, 2014 and at times prior thereto, the employer did not establish and implement a written respiratory program to include worksite procedures, procedures for selecting respirators, medical evaluation of employees, training of employees in the hazards, proper use, maintenance, and any limitations of respirators which exposed employees to respiratory hazards such as but not limited to obstructed breathing. The employer provided, required and/or allowed voluntary use of NX/N95 Particulate Respirators.
