29 CFR 1926.452(n)(1): Scaffold platforms were placed higher than the second highest rung or step of the ladder supporting the platform. (a) Raymond J. Schaefer, Inc. on or about March 24, 2014, at the address of 627 Cedar Point Rd. Sandusky, OH: The employer failed to ensure the bearing end of a scaffold platform was supported below the top step of a 10 ft. step ladder. An employee performing soffit work form the scaffolding platform position on the top of the self-supporting ladder exposed the employee to overturning hazards and ultimately a 10 ft. fall to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: (a) Raymond J. Schaefer, Inc. on or about March 24, 2014, at the address of 627 Cedar Point Rd. Sandusky, OH: The employer failed to ensure the top of a 10 ft. self-supporting ladder was not used as a step. An employee was utilizing the top of the ladder as a working surface to reach the eave level during soffit installation work. The employee was exposed to fall hazards due to improper ladder use.
