Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to crushing hazards due to being struck by a collapsing industrial steel storage rack:   A.  On or about December 20, 2013, Pre-Fab Shop at 145 Robert Jackson Way, Plainville, Connecticut:  Employees were exposed to crushing hazards whlie loading spooled wire onto an industrial steel pallet rack.  The rack had been modified by the employer with the addition of wheel casters to allow the rack to be moved throughout the shop.  Spools were loaded onto the front side of the rack by four employees.  The rack was not securely affixed to the concrete shop floor, and without counterbalance, the top-heavy rack collapsed, resulting in serious injury to one of the employees.
