29 CFR 1910.215(a)(2): The safety guard did not cover the spindle end, nut, and flange projections: Hand Polishing Areas, Jack Lathe Polishers: Where employees were using abrasive wheels to polish large metal castings, the employer did not place safety guards over non-occupied rotating shaft ends and spindle/nut connections, and this exposed the employees to laceration and amputation hazards from open and unguarded rotating parts.
