29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  922 South 20th Street, Philadelphia, PA- An employee cutting out flooring worked on the remaining section of the second floor while exposed to fall hazards of approximately twenty feet to the basement without any form of fall protection. Observed on or about March 21, 2014.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  922 South 20th Street, Philadelphia, PA- An employee descended from the remaining section of the second floor to the remaining section of the first floor by sliding down a wooden joist propped against the party wall of the house.  There was no ladder or other form of safe access in place. Observed on or about March 21, 2014.

29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations:   a) 922 South 20th Street, Philadelphia, PA- Employees were exposed to potential collapse hazards while performing manual demolition activities in that there was no evidence in writing that an engineering survey was performed by a competent person for the demolition work that was well underway and nearing completion. Observed on or about March 21, 2014.    A & M Curran LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.850(a), which was contained in OSHA inspection number 312492804, citation number 1, item number 3a and was affirmed as a final order on March 2, 2010, with respect to a workplace located at North 2nd Street and West York Street in Philadelphia, Pennsylvania.
