29 CFR 1910.134(k)(1): The employer did not provide respirator training that would ensure each employee could demonstrate knowledge of items in section (i)-(vii): (Construction reference 1926.103) (a) Great American Builders Inc. did not provide effective training on the use of respirators that included the requirements of (k)(i) and why the respirator is necessary and how improper fit, usage, and/or maintenance can compromise the protective effect of the respirator. (b) Great American Builders Inc. did not provide effective training on the use of respirators that included the requirements of (k)(iv) covering how to inspect, put on and remove, use, and check the seals of the respirator. (c) Great American Builders Inc. did not provide effective training on the use of respirators that included the requirements of (k)(vii) the general requirements of the respiratory standard.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103) (a) An employee of Great American Builders Inc. was provided a half mask tight-fitting negative pressure respirator which was being worn over a full beard.
