29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Mill Department: On or about March 10, 2014 employees were not de-energizing the hammer mills by deactivating the circuit breaker and applying a lock prior to removing protective guards to do preventive lubrication of the rotating hammer shaft as determined on March 25, 2014.
