Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 1370 Lancaster Avenue, Columbia, PA - The employer allowed employees to engage in roofing work on a 6 in 12 pitch roof without using a fall protection system. The employees were exposed to a fall of approximately 9 feet 11 inches to the ground below, on or about March 18, 2014. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 1370 Lancaster Avenue, Columbia, PA - The employer allowed employees to use an Aluminum Extension Ladder that did not extend at least 3 feet above the roof landing at the front the building. Employees were exposed to a fall hazard of approximately 9 feet 11 inches to the ground below, on or about March 18, 2014. No abatement certification or documentation required.
