29 CFR 1926.403(i)(2)(i): Employer did not ensure live parts of electric equipment operating at 50 volts or more were guarded against accidental contact by cabinets or other forms of enclosures. (a) 6 Platt Street, Every Floor by Elevator Lobby. Employees working on the 10th and 21st floor were not protected from electric shock by accidental contact with energized parts in load centers. Circuit breakers parts and bus bars were exposed because covers had not been installed on the energized panel boxes. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employees working throughout building were not protected from electric shock or burns from accidental contact with broken or exposed lamp sockets. Numerous temorary lamp sockets in hallways and rooms were not protected with a protective cover or cage. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

Temporary lights for general illumination were installed on the walls of the hallways mounted about 6 feet above the floor. The temporary lights were suspended by their electric wires and not provided with any strain relief. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. (a) 6 Platt Street, 10 th floor outside elevator lobby. Employees working in hallways outside elevator lobby were exposed to electrical shocks due to an branch circuit wire running not being installed in junctoin boxes with covers. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
