29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that th(a)(LOCATION)(DIMENSIONS OF THE EXCAVATION)(EMPLOYEES EXPOSED)(THE EMPLOYER HAD NOT MET ALL THE ELEMENTS OF SLOPING/BENCHING OPTION (2), -3 OR (4))NOTE: THIS OPTION IS FOR USE WHEN SLOPING IS INADEQUATE. SEE CPL 2.87, DATED FEBRUARY 20, 1990, REQUIREMENTS FOR PROTECTIVE SYSTEMS, (d)(1)(a) AND (b), PAGE 13    (a)  An employee in an approximately 9 foot trench for the purpose of installing a sewer lateral, was not protected from cave-ins with an adequate protective system.  This exposed employees to a trench collapse hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)    (a)  The portable ladder being used for entering and exiting the trench did not extend at least 3 feet above the upper landing area.
