29 CFR 1917.95(b)(3): The employer did not provide employees with personal flotation devices that were maintained in a safe condition or that were considered unserviceable when damaged so as to affect buoyancy or fastening capability.  a) Dock - On or about 3/26/14, employees were exposed to drowning hazards when life vests contained cracks and damaged to outer shell.
