29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides.    The employer does not protect employees from falling when working from open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level.   This violation was observed on or about March 25, 2014, in the mold process area, where employees were exposed to falls of 4 feet or more above the ground level when working from the unprotected edge of an elevated platform.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  employees are protected from falling when exposed to falls while working on open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   The employer does not provide employees with a program consisting of energy control procedures, training, and periodic inspections for employees conducting lockout/tagout activities.  This violation occurred on or about March 25, 2014, in the molding area where employees were exposed to electrical, caught-between and stuck-by hazards while performing service and/or maintenance and setup to the Rotational Molding Process without a lockout/tagout program.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a program is in place consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs servicing or maintenance on a machine or equipment.

29 CFR 1910.219(f)(3):Sprocket wheels and chains that were seven (7) or less feet from the floor or platform were not enclosed:   The employer does not enclose sprocket wheels and chains that were less than seven (7) feet from the floor or platform.  This violation was observed on or about March 25, 2014, on the right side of the elevating platform, where employees were exposed to caught-in hazards when changing out molds on the mold machine when the sprocket and chain was less than seven (7) feet from the floor and not enclosed.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all sprocket wheels and chains that were seven (7) or less feet from the floor or platform are enclosed.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer does not ensure that employees who operate powered industrial trucks are competent to operate the powered industrial trucks as demonstrated through completion of training and evaluation as specified in paragraph (l).  This violation occurred on or about March 25, 2014, and at times prior thereto, when employees operated powered industrial trucks without the employer ensuring the operators were competent by completion of training and evaluation.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure they provide employees who operate powered industrial trucks with training and evaluation.
