The employees were exposed to tripping hazard or fire hazard if an evacuation was required. b. Facility: The employer moved to a new location and the employees were required to use a Toyota, Model 7EDU45, which is different from the forklifts at the previous site. Employees were not trained on the new forklifts prior to operating them. The employees were exposed to struck by and crushed by hazards.

29 CFR 1910.1026(d)(2)(i): The employer using the scheduled monitoring option did not perform initial monitoring to determine the 8-hour time-weighted average exposure to chromium (VI) for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area: a) Warehouse: The employer did not conduct monitoring for hexavalent chromium for employees who are welding stainless steel. The employees are potentially exposed to hazardous fumes classified as chromium VI, which produces carcinogens. CSHO monitored the area on 5/22/14. The results of sampling showed hexavalent chromium (VI) was not detected.
