29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  a) At the facility, on or about March 26, 2014 - Employees, who are required to wear a Sperian Full-facepiece respirator when painting, were not provided with an annual fit test.   Starflite Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 445419, citation number 1, item number 4d and was affirmed as a final order on November 23, 2012, with respect to a workplace located at 116 Dogwood Lane, Connoquenessing, PA 16027.
