29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Building #1, Machine Shop, on or about 3/26/14: Chas Allen Co. drill press was not equipped with a guard to prevent contact with the serrated chuck and tool. b) Building #3, on or about 3/26/14: Ridgid pipe threading machine, model 535, was not equipped with a momentary contact device, such as a foot switch, to control the power input to the machine. c) Building #3, on or about 3/26/14: Palmgren radial arm drill was not equipped with a guard to prevent contact with the serrated chuck and tool.
