29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems. Location: 605 W. 42nd St., New York, NY. a) Employees used the crane's tower and/or mast to access their work zone. There was a gap of approximately 2 ft. between the edge of the concrete slab/floor and the mast. Employees were exposed to a potential fall of up to approximately 20ft to the floor below; on or about 3/20/14.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section. Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used. Location: 605 W. 42nd St., New York, NY. a) Employees used the crane's tower and/or mast to access their work zone. There was a gap of approximately 2 ft. between the edge of the concrete slab/floor and the mast. Employees were exposed to a potential fall of up to approximately 20ft to the floor below. Employees demonstrated inefficiencies by means of actions. Fall protection re-training is required; on or about 3/20/14.
