The employee's eyes and faces were exposed to contact with a caustic liquid, on or about March 26, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Building 100 and Building 200, 50 Rausch Creek Road, Tremont PA - Employees operated the Raymond Standup Counterbalanced Truck Model 425-C35TT (Serial No. 425-12-34720) with a front-end attachment Cascade Carton Clamp Model 25D-CCS-35Q (Serial No. PTL1805064-1R4) without the manufacturer's prior written approval. Employees were exposed to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, as observed on or about March 26, 2014. b) Building 100 and Building 200, 50 Rausch Creek Road, Tremont PA - Employees operated a Raymond Standup Counterbalanced Truck Model 425-C35TT with a front-end attachment Cascade Push/Pull Model 35E-QPB-Q002R1 (Serial No. 225625-1R1) without the manufacturer's prior written approval. Employees were exposed to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about March 26, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
