29 CFR 1910.212(a)(3)(ii): The employer did not ensure that a point of operation machine whose operations exposed employees to injury were guarded: a) Top Metal Buyers, Inc. @ 2007 Southern Expressway, Cape Girardeau, MO 63701: On and before March 26, 2014, the employer did not ensure that employees were protected from a point of operation hazard. The vertical baler was equipped with a damaged screen that exposed employees to flying debris. The damage was located along the base of the screen and measured as large as 4 inches in length by 4 inches in height, in some places.
