29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, Norwin Towne Square, Irwin, PA: On or about 03/24/14 an employee was working from a steel beam nailing rafters into place and was not protected from falling approximately 23 feet to the ground below. The employee was wearing a harness with a lanyard attached but there were not anchorage points in place for the employee to tie off to nor were any guardrail systems or safety net systems in place to provide fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, Norwin Towne Square, Irwin, PA: On or about 03/24/14 the employer did not provide training on fall protection for employees who had potential exposure to falls in the course of their work. Employees were working at heights up to 23 feet without fall protection in place indicating that adequate training on fall hazards and procedures to prevent falls had not been provided to ensure employees worked safely.
