29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) Shop: On or about March 26, 2014 and at times prior thereto, when the employer required employees to wear a respirator such as but not limited to a 3M 5300 half-face respirator with organic cartridges, the employer did not ensure employees were provided with and/or implement an effective written respirator program that included the following provisions: 1) The program is not worksite specific and did not list the types of respirators used or the program administrator. 2) The employees had not been medically evaluated to wear a respirator. 3) The employees had not been fit tested annually on the tight fitting respirators.
