29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, and the employer could not demonstrate that a wider space was necessary:    a) Scaffold On Route 61 Side Of Building - The working level of the tubular welded frame scaffold was not fully planked or decked between the front and rear uprights thus exposing employees to a fall hazard of approximately twelve (12) feet, on or about March 27, 2014.    No abatement certification or documentation required.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:     a) Scaffold On Route 61 Side Of Building - An employee climbing the tubular welded frame scaffold was exposed to a slip, trip or fall hazard from a step of approximately thirty-two (32) inches to thirty-six (36) inches in height, on about March 27, 2014.    No abatement certification or documentation required.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a) Scaffold On Route 61 Side Of Building - An employee replacing mirror panels on the front window frame from a tubular welded frame scaffold was exposed to fall hazards of approximately twelve (12) feet without the use of personal fall arrest systems or guardrail systems, on or about March 27, 2014.    No abatement certification or documentation required.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the  safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) In Front of Building, Off Route 61 - Employees were not trained on the explanation of the safety data sheet, including the order of information and how employees could obtain and use, on or about March 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
