29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:     (a)    Location: 155 6th Street, Chelsea, MA   On or about 3/14/14, the employer did not instruct employees in the recognition and avoidance of unsafe conditions while working with a portable table saw.

29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:Note:  See paragraph (i)(1) of this section for additional requirements for hand-fed crosscut table saws.     (a)    Location: Unit 535  On or about 3/14/14, employees were not protected from injuries when working with a table saw that had the guard removed while cutting laminate wood flooring.
