Occurred on or about 03/24/2014. b) The employer failed to provide documentation and certification for abatement of citation 1 item 1b in which: An employee was observed working from a work platform on the raised forks of a rough terrain fork lift (RTFL). The worker was not protected by guardrails, fall restraint, PFAS or any other form of fall protection and was thus exposed to fall hazards of greater than 16.5 feet from the working surface to the ground. Occurred on or about 03/27/2014.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
