29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about 03/27/2014, at the job site - the use of personal fall protection was not enforced while an employee was applying stucco from a roof, exposing the employee to an 11 feet fall hazard.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a. On or about 03/27/2014, at the job site - a worker was exposed to 14 feet 5 inches, 7 feet 11 inches, and 3 feet 6 inches fall hazard, in that, the scaffold was not fully planked.    B & K Stucco Exterior Inc. was previously cited for a violation of the occupational safety and health standard which was contained in OSHA inspection number 313878548, citation number 1, item number 1, issued on 11/08/2010, and was affirmed as a final order on 12/06/2010, with respect to a workplace located on 11901 Atlantic Blvd., Jacksonville, FL 32225.
