29 CFR 1910.215(a)(4): Work rest on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:     (a) On or about March 27, 2014, and at times prior, Waste Connections of Colorado, Inc. did not ensure that work rests on grinding machinery was adjusted closely to the wheel with a maximum opening of one eighth inch. Employees were exposed to struck-by hazards, in that an abrasive wheel bench grinder was observed in the repair shop with an opening of one quarter inch between the work rest and the wheel. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:     (a) On or about March 27, 2014, and at times prior, Waste Connections of Colorado, Inc. did not ensure that adjustable tongue guards on grinding machinery was adjusted closely to the wheel with a maximum opening of one quarter inch. Employees were exposed to struck-by hazards, in that an abrasive wheel bench grinder was observed in the repair shop with an opening of three quarter inch between the tongue guard and the wheel. Employees in the repair shop were exposed to health hazards while wearing half-face respirators. Employees in the repair shop were exposed to health hazards while wearing half-face respirators without being fit tested.
