29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 26, 2014, and times prior thereto, the unused portion of the blade on the Cosen band saw was not guarded. Employees were exposed to the hazard of being struck by saw blade.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about March 26, 2014, and times prior thereto, the employer did not develop, implement and maintain a written hazard communication program that described and included information on labeling, material safety data sheets for chemicals being used, and training. Employees used products that contained chemicals such as, but not limited to, phosphoric acid in concentrations of 1-10% and manganese dihydrogen phosphate at concentrations of 10-30%. Employees were exposed to respiratory and dermal hazards.
