The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a. On or about March 28, 2014, in the facility, the employer did not maintain OSHA 300 logs for the current year, 2013, 2012 and 2011.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: a. On or about March 28, 2014, in the facility, employees were exposed and required to work with hazardous chemicals including, but not limited to urea, ammonium sulfate and carbon monoxide. The employer failed to develop and implement a written hazard communication program.
