Mitchell Construction Contractors LLC worksite located at 3610 Butz Rd., Maumee, Ohio: On or about March 26, 2014, the employer did not ensure employees were protected from head injuries while working below employees installing weatherproofing. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

The employees were exposed to a fall distance of approximately 13.00 feet while working on the scaffold. No form of fall protection was in place.
