29 CFR 1910.119(e)(3)(vii): The employer did not ensure that the process hazard analysis (PHA) had a complete qualitative evaluation of a range of the possible safety and health effects of failures of controls: 1214 Road G, Fairmont, NE 68354: The employer is failing to ensure that the process hazard analysis (PHA) had a complete qualitative evaluation of a range of the possible safety and health effects of failures of controls. The process hazard analysis (PHA) was completed on April 26, 2012. However, the PHA did not fully address qualitative evaluation of a range of the possible safety and health effects of failures of controls including, but not limited to; having an incomplete evaluation on the PHA Unit: Ammonia Unloading. There was an evaluation of the unloading of the wrong material; however, the consequences in regard to the possible injury did not detail what the actual injuries could be. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
