29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about March 25th, 2014, at a job site located at 19019 48th Street in Independence, MO, a worker was exposed to a fall hazard while using an A-frame step ladder to access an upper landing surface in that the side rails did not extend at least 3 feet above the upper landing surface. Abatement certification for this Item is not required.
