29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  At the Shoup Plant F-3 filter inlet separator unit, a hazard assessment was not performed for all potential hazards employees would have been exposed to which necessitated personal protective equipment such as, but not limited to, tight fitting respirators. Employees were exposed to hazardous vapors while removing and replacing filters.

29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer:  At the Shoup Plant of the host employer, employees engaged in removing 96 filters from the F3 inlet filter separator unit were exposed to a permit space which contained matter that ignited spontaneously.  The employer did not obtain all information regarding permit space hazards to inform its employees of hazards, such as, but not limited to, the scale and sludge residue from the processing of natural gas igniting and/or exploding once exposed to air.

29 CFR 1910.132(h)(1): Except as provided by paragraphs (h)(2) through (h)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employees.   Employees are not provided with personal protective equipment such as fire resistant clothing at not cost to the employees. Employees are charged for fire resistant clothing purchasing and laundering by payroll deduction. Employees do not use the personal protective equipment, fire resistant clothing, away from the work site.
