29 CFR 1926.20(b)(2):  Employer did not provide programs for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer:            (a)  Colorado Superior Roofing and Construction at 1717 East 39th Ave., Denver, CO 80206: On and before 3/14/14, the controlling employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person.  Employees on-site were permitted to work on the roof without fall protection. This condition exposed employees to a fall hazard of approximately 20 feet.     Abatement Note: OSHA defines a competent person as one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(1): Each employee on a walking working surface (horizontal and vertical surface) with an unprotected side and edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)  Colorado Superior Roofing and Construction at 1717 East 39th Ave., Denver, CO 80206: On and before 3/14/14, the controlling employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected side and edges of a roof on a commercial building. Employees were exposed to falls in excess of 20 feet.   Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladders length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder:      (a)  Colorado Superior Roofing and Construction at 1717 East 39th Ave., Denver, CO 80206: On and before 3/14/14, the employer did not ensure that employee was utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employee was using a portable extension-ladder for access to the roof of a commercial building. Employees were exposed to a fall of approximately 20 feet fall while attempting to use the ladder.
