29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): a) Jobsite, 3101 Brownsville Road, South Park, PA 15129: Observed on or about March 28, 2014, the employer did not establish an energy control program to protect employees form the hazards associated with the unexpected startup or energization of machines and equipment used at the facility. Employees engaged in maintenance work on a shredder/trommel (model number 621T, serial number CJD1703) were exposed to caught-by and crushed-by hazards.
