29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a) Roof of 6892 Tara Boulevard, Jonesboro, Georgia. An employee, removing external metal conduit housing electrical wiring, was exposed to fall hazards of approximately 17-feet while working from an unprotected low-sloped roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.     a) Roof of building at 6892 Tara Boulevard, Jonesboro, Georgia. An employee was exposed to fall hazards of approximately 17-feet while mounting and dismounting an aluminum extension ladder. The side rails of the ladder did not extend at least 3-feet above the upper landing being accessed.

29 CFR 1926.1053(b)(4): A ladder was used for purposes other than the purpose for which they were designed:     a) Roof of building located at 6892 Tara Boulevard, Jonesboro, Georgia. An employee working from a self-supporting ladder, being used in a way that it was not designed for, was exposed to fall hazards of approximately 17-feet. The self-supporting ladder, which had not been opened and locked into place, was leaning against a wall.
