29 CFR 1910.106(b)(6):  In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition included open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    At Shannon EFS 1H site, contractor employees connected, disconnected, and cross-checked electrical connections of the cables to two batteries in an area where sparks or other ignition sources could serve as a potential ignition source for flammables vapors in the area.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    On March 15, 2014, and times prior thereto, at Shannon EFS 1H site, the employer did not conduct a hazard assessment for personal protective equipment and certify that the required assessment had been performed.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  At Shannon EFS 1H site, the employer did not evaluate for the presence and ambient levels of air contaminants such as flammable vapors or "flash gas" for employees such as but not limited to those performing maintenance or repair work.  Employees were exposed to inhalation and explosion hazards.
